Enrollment is based on residency, not on the elementary school your student attended.  Use this Boundary Locator to see which middle school is yours.

The Registrar Office will be open during Summer for any parent who needs help registering their child. Registration begins online.

2023-2024 Online Enrollment - follow this link to begin the online enrollment process. You must complete this process before the Registrar at the school can enroll your child in the school.

 

Required Enrollment Documents - Documents that must be uploaded when enrolling student

▢  Verification of Identity for Parent (Drivers Licence, passport, etc

▢  Student Proof of Age (Birth Certificate, passport, etc)

▢  Current SDGE Bill or Letter of Residency

▢  Residency Verification from Category 2 (current Water Bill, Cable Bill, Internet Bill, Tax Document, Pay Stub or Social Services document that shows the residential address)

▢  Proof of Students Immunizations

▢  Documents from previous school (Report Card w/ course history)

Student Health Information Form - will need to be completed and uploaded when enrolling student

Middle School Course Profiles and Descriptions 

MDTP Testing - required Math testing for students new to our District. This test will be offered at PTMS on the following dates: June 16, 23, and 30. July 19, and 28. August 2. Please follow this LINK to sign up for a test date.

 

IMBHRT Information - The SDUHSD IMBHRT is an optional test for incoming 7th grader who are advance in Math and who wishes to enroll in Integrated Math B Honors in 7th grade. They must take the SDUHSD IMBHRT offered by the District and score a 70%or higher.
